Appointment Checklist

1. Current Glasses and Contact Lenses

Please bring with you any glasses and contact lenses that you are currently using, including sunglasses and computer glasses.

2. Previous Written Prescriptions

If your last examination was not with us, please bring a written copy of any prescriptions that you were given at your last exam. Contact lens wearers, if you are unable to find a written contact lens prescription, the contact lens boxes, blister packs, or previous order history will suffice.
